Scope of Work:
- Supervising, monitoring & controlling the vaccination program within the Qadaa dispensaries & Primary Health Care Centers.
- Coordinating the mechanism of receiving and submitting the vaccines to the entitled dispensaries & Primary Health Care centers, hospitals and some private physicians within the Qadaa per the guidance set by the EPI management.
- Monitoring the proper functioning and effectiveness of the cold chain system within each dispensary & Primary Health Care centers in the Qadaa;
- Ensuring that the validity of the documented data is pertinent and relevant to the vaccination process through regular revision of the reporting received.
- Coordinating & monitoring data entry concerning all vaccination processes.
- Conducting periodic field visits to dispensaries & Primary Health Care centers in the Qadaa to audit the documentation process, & cold chain.
- Building a team spirit and sense of interdisciplinary cooperation with the Qadaa Physician.
- Documentation of EPI activities per the guidance of EPI management.
- Perform other duties as required by PHC Department.
- Reporting Requirements The consultant will be reporting to the Director of the PHCC Dept, Ministry of Public HealthA detailed monthly report must be submitted to UNICEF, on the development and progress of the project, with all the related deliverables.

Minimum Qualifications Required: management If Other, Enter Disciplines: Years of Experience required: 3+ Knowledge/Expertise/Skills required: - Bachelor’s degree in any field.
- Any experience in the field of procurement, logistics or supply chain or a related area is a plus.
- Previous knowledge of vaccine cold chain management is a plus.
- Strong organizational and planning skills.
- Excellent communication and interpersonal skills.
- Language: Fluency in both oral and written Arabic and English.
- Holder of a Lebanese Nationality.

Deliverables/Outputs: Tasks/Milestone: Timeline Percentage of Payment (payments can be divided per 1 or more deliverables) · Submission of monthly reports detailing vaccination activities, cold chain status, and data accuracy by the 5th of each following month. - Collect vaccination activity data from all relevant health facilities.
- Verify completeness, timeliness, and consistency of data submitted.
- Assess cold chain equipment functionality, temperature records, and any incidents.
- Analyze data for trends, coverage gaps, and discrepancies.
- Draft and compile the monthly report including key findings, challenges, and recommendations.
- Submit the finalized report to MoPH and UNICEF by the 5th of the following month.
Monthly · Comprehensive reports on field visits conducted, including findings and recommendations, submitted by the end of each quarter. · Develop and maintain a quarterly field visit plan (including sites, dates, and objectives). · Conduct on-site supervision of vaccination sessions, cold chain equipment, and data recording practices.
· Interview staff at the facilities to assess capacity, training needs, and operational challenges.
· Collect evidence (photos, temperature logs, maintenance records) during visits.
· Analyze observations and identify areas needing corrective action or support.
· -Prepare a consolidated quarterly field visit report summarizing findings, gaps, and actionable recommendations.
· Submit the comprehensive report by the end of each quarter to MoPH and UNICEF.
Monthly · Submit audit reports on the cold chain system’s effectiveness in each dispensary and Primary Health Care center after each field visit. · Use a standardized cold chain audit checklist during each site visit. · Inspect vaccine storage areas, temperature monitoring, equipment maintenance logs, and emergency procedures.
· Assess compliance with vaccine handling and cold chain standards.
· Identify risks to vaccine potency and propose mitigation measures.
· Draft a site-specific cold chain audit report immediately after each visit, including clear recommendations and timelines for corrective action.
· Submit the audit report to MoPH and UNICEF within the agreed timeframe post-visit.
Monthly · Monthly validation reports on the accuracy and relevance of immunization data received from health facilities. · Collect immunization data from health facilities on a monthly basis. · Cross-check reported data with source registers and vaccination cards to verify accuracy.
· Identify and document discrepancies, errors, or data quality gaps.
· Provide feedback to facilities to improve data reporting and accuracy.
· Prepare a monthly validation report summarizing the findings and any corrective actions taken.
· Submit the validation report to MoPH and UNICEF alongside the monthly report.
